Btw- the recall on cybex smith models like in HC gym is a bolt-on modification by cybex that converts it to a safety hook mechanism like the newer cybex version. I have the same cybex smith as hc gym, and have also seen one upgraded and to be honest don’t see the point. The recall was about people not having the hook part fully engaged to the ladder style racking pins- then the bar dropping on them.
I think the biggest problem with many smith machines is people not setting the safety stops- usually you see them setup at the bottom or floor. Think of the smith as a rail car on vertical train tracks. If you fail on a bench press on freeweights, you can dump the bar to one side(assuming no clamps), with the smith there is no dumping the weight.
